Preparing For Your Admission
Bookings
- Your booking will be made through your doctor’s surgery.
For a smooth admission process, we ask that you attend
to the following matters prior to your admission:
- Please complete the Hospital Registration Form and
both sides of the Patient History Sheet.
- Return both completed forms to the hospital as soon
as possible. If returning the forms by fax could you please
bring the original
forms when presenting for admission.
- If you return the forms by mail or fax, you may be
contacted by telephone if the hospital needs to confirm your
details.
- The Pre-admission nurse will contact you to discuss
your patient history details.
- You may be requested to attend the hospital to be
seen by the pre-admission nurse and /or anaesthetist. If you
are notified
to attend the Pre-admission Clinic.
Medications
- Please list medications you are taking on the Patient History
Sheet (including vitamins & herbal remedies).
- Unless instructed otherwise by your doctor or the
pre-admissions nurse, continue to take your usual medications.
- Bring in to hospital your normal medications in their
original packaging, including asthma puffers.
- Ensure that you have sufficient medications or repeat
prescriptions for at least approx 1 week after discharge.
- Medication pertaining to existing conditions or medicines
ordered as part of your discharge plan will be dispensed by
a local Pharmacy.
The pharmacy would appreciate payment being finalised
on discharge, cash, cheque or credit card payment is accepted.
Hygiene
You are requested to have a shower on the morning of your admission.
- Do NOT use talcum powder or perfume.
- Do NOT wear any make up.
- Do NOT clip / shave the hair from the area to be operated.
- Remove any nail polish from fingernails and toenails.
- Depending on the type of surgery, you may be requested
to have a shower on arrival to hospital with a special pre-operative
body wash.
DO NOT bring to hospital
- Jewellery, valuables or large sums of money
- Mobile phones.
Medical Certificate
Notify your doctor if you require a medical certificate.
Admission and Fasting Times
The pre-admission nurse will notify you of the time you are to
arrive at the hosptial and when you are to fast.
NO SMOKING & NO ALCOHOL for at least 24 hours before your
procedure.
Day Surgery Patient Information
The Preadmission nurse will contact
you prior to your admission.
You must arrange for a responsible adult to accompany you home
and to remain with you till the following morning. You must not
drive a vehicle for 24 hours.

What To Bring
- Department of Veterans Affairs card
- Pension Entitlement card
- Pharmaceutical Entitlement card
- Relevant existing X-rays and CT scans
- All medications you are currently taking in their
original container (including tablets, vitamins, mixtures,
creams, inhalers or drops).
If possible, please bring a list from your General
Practitioner of your current medications, their strengths and
dosages.
- Sleepwear, dressing gown for overnight stay and slippers
or comfortable shoes (not necessary for Day Procedure Patients)
- Gynaecology patients - Sanitary napkins
- Dental procedures - box of tissues
- Glasses, hearing aids and non-electronic walking
aids
- Toiletries, including soap, toothpaste and toothbrush
(not necessary for Day Procedure Patients)
- Reading material
Fees and Charges
Prior to your admission please
contact your private health insurance fund to verify your level
of cover.
- Please confirm that you are covered for the procedure
as some levels of cover have exclusions. This may require you
to
obtain a proposed procedure item number from your surgeon.
- Is there an excess, how much and does it apply to day
cases and/or overnight stays?
- Are there bed fee/co-payments, how much and do they apply
to day cases and/or overnight stays?
If you do not have private health insurance, you may need to
obtain the proposed item numbers from your surgeon to enable
the hospital to prepare an estimation of your hospital account.
Please confirm this with the hospital when you lodge your admission
forms. The estimated cost is payable on admission by cash, cheque
or credit card.
